How to Request Compounded Ivermectin or Mebendazole

Ready to request your prescription?

The secure intake form takes about five minutes. Tell us whether you are interested in ivermectin, mebendazole, or the ivermectin/mebendazole combination, and our Fort Worth pharmacist will review your request and follow up with next steps. Texas residents only.

1. Complete the secure prescription intake form. Tell us which medication you are interested in (ivermectin, mebendazole, or the ivermectin/mebendazole combination), your health history, and your current medications, then pay for the physician visit ($75, or $105 with labs). Texas residents only.

2. Pharmacist and prescriber review. Our team reviews your request, confirms the medication and strength are appropriate for you, and, if the prescription is approved, contacts you to pay for your medication. You are never charged for medication before approval.

3. Compounded in Fort Worth and delivered. Your capsules are prepared in our compounding lab and shipped anywhere in Texas, or held for in-store pickup at 2410 Forest Park Blvd.

About Compounded Ivermectin and Mebendazole

Two well-established antiparasitic medications. Ivermectin is an FDA-approved antiparasitic that has been prescribed for decades for conditions such as strongyloidiasis and onchocerciasis, and since Texas House Bill 25 it is also sold over the counter in Texas as a 3 mg tablet. Mebendazole is an FDA-approved anthelmintic used to treat intestinal worm infections including pinworm, roundworm, hookworm, and whipworm. Both are on the World Health Organization's list of essential medicines.

Why choose compounded capsules? Commercial ivermectin and mebendazole come in a limited number of fixed strengths. Compounding lets a licensed pharmacist prepare capsules to the exact dose your prescriber orders, combine both ingredients into a single ivermectin/mebendazole capsule when requested, and leave out dyes or fillers a patient needs to avoid. Every batch is prepared in our Fort Worth compounding lab under USP <795> standards for nonsterile compounding.

Ivermectin/mebendazole combination capsules. Some prescribers order ivermectin and mebendazole together for patients who would otherwise take two separate products. A single compounded capsule can simplify the regimen and lower the cost. Because this combination is not commercially manufactured, it can only be prepared by a compounding pharmacy from a valid prescription.

Cost-plus, PBM-free pricing. Forest Park Pharmacy does not bill through pharmacy benefit managers. Compounded ivermectin is $55 per 30 capsules, mebendazole is $120 per 30 capsules, and the ivermectin/mebendazole combination is $130 per 30 capsules. If you need a prescription, you pay for the physician visit first ($75, or $105 with labs), then pay for your medication only if the prescription is approved. Pay by card, cash, or an HSA/FSA card.

Important: Compounded medications are prepared for an individual patient based on a prescriber's order. They are not FDA-approved products and have not been evaluated by the FDA for safety or effectiveness. Off-label uses discussed online have not been established in large clinical trials. Talk with your prescriber about whether these medications are appropriate for you.

Frequently Asked Questions

Do I need a prescription for compounded ivermectin or mebendazole? Yes. Both are prescription-only when compounded. If you already have a prescription, your prescriber can send it to Forest Park Pharmacy in Fort Worth, or you can complete our online intake form and our pharmacist will coordinate with a licensed Texas prescriber (you pay for the physician visit first, $75 or $105 with labs, and pay for medication only if the prescription is approved). Ivermectin 3 mg tablets are also available over the counter in Texas without a prescription.

Who can order? Texas residents only. We ship compounded medications anywhere in Texas and offer in-store pickup at our Fort Worth pharmacy near TCU, Arlington Heights, and the Fort Worth Medical District.

How long does it take? Most intake requests are reviewed within 1 to 3 business days. Once a valid prescription is on file and you approve your order, capsules are typically compounded and shipped or ready for pickup within a few business days.

What strengths and forms are available? Strengths are set by your prescriber. Because we compound to order, we can prepare ivermectin capsules, mebendazole capsules, or ivermectin/mebendazole combination capsules in a wide range of doses, and we can leave out dyes, lactose, or other excipients on request.

What are the possible side effects and precautions? Ivermectin is generally well tolerated at prescribed doses; the most common side effects are dizziness, nausea, diarrhea, and itching or skin rash. Mebendazole may cause abdominal pain, nausea, or diarrhea, and prolonged or high-dose use has rarely been associated with elevated liver enzymes or low blood counts, so your prescriber may recommend periodic lab monitoring. Tell your pharmacist about all medications you take, especially warfarin and anti-seizure medications, and let us know if you are pregnant, breastfeeding, or have liver disease. This is not a complete list of side effects or interactions.

Can I just buy ivermectin over the counter instead? Yes, for the standard 3 mg tablet. Texas law allows pharmacies to sell ivermectin tablets without a prescription, and you can order OTC ivermectin from our online store for delivery or pickup. Compounded capsules, custom strengths, mebendazole, and the ivermectin/mebendazole combination still require a prescription.

Is Forest Park Pharmacy a licensed compounding pharmacy? Yes. Forest Park Pharmacy is an independent, LegitScript-certified pharmacy licensed by the Texas State Board of Pharmacy. Our compounding lab prepares nonsterile capsules under USP <795> standards, and every prescription is checked by a pharmacist before it leaves the building.
