Nobody Shops for an Ambulance. Here's What a Ride Cost.

A picture of an Ambulance

You're bleeding in the back of an ambulance. Nobody hands you a price list.

You don't shop. You don't compare. You ride, and the price shows up later. Ambulance bills are all over social media right now, so I went looking for real numbers instead of viral ones. The best long-term look I found is a data brief from the Health Care Cost Institute. It covers 2012 to 2021 and was published in 2023, so it isn't new, but it shows the trend clearly.

What HCCI Found

HCCI studied ambulance trips for people under 65 with health coverage through an employer. Its sample included 8.5 million ground trips and 175,000 air trips. Ground ambulances made up more than 98% of trips.

Average per trip

Ground ambulance out-of-pocket cost

Helicopter ambulance price

$16,385

$30,151

Airplane ambulance price

The out-of-pocket number is the one I'd look at twice. The part people paid themselves grew almost twice as fast as the price of the ride.

You Don't Pick the Ambulance

HCCI found that 35% of ground ambulance trips were billed out of network in 2021. That's down from 43% in 2012, and most of the drop came between 2020 and 2021. It's still more than one in three rides.

There's a reason for that. In an emergency, you don't choose who comes. The ambulance picks you.

The One Time You Can't Ask

Even the rules built to give people a price ahead of time stop at the emergency room door. According to CMS, people who don't have or aren't using insurance can usually get a good faith estimate of charges before care, but you won't get one during emergency care.

So an emergency really is the one time you can't ask the price.

Your Refill Isn't an Emergency

Most of what you pay for at a pharmacy is the opposite of an ambulance ride. A refill is planned. It repeats every month. You know it's coming, and you have time to look up what it costs.

That's the whole reason we post our prices. You shouldn't have to find out at the register.

Questions People Ask

How much does an ambulance ride cost? HCCI found the average ground ambulance trip price for people with employer coverage was $1,093 in 2021, up from $820 in 2012. Your own bill can be very different.

Is this new data? No. HCCI's brief covers 2012 to 2021 and was published Oct. 12, 2023.

Can I get a price before an ambulance ride? Usually not. CMS says good faith estimates aren't provided during emergency care.
